2026 School Sports Physicals
Step 1: Schedule Your Appointment
Use the QR Code or link provided to register for an appointment at Public Health on June 2nd OR July 30th.
- This visit will complete the first portion of the physical.
- Only ONE appointment is needed.
- Immediately after your Public Health appointment, your student will go directly to the clinic for their exam with a provider at FMC (Fallon Medical Complex) to complete the 2nd portion of your physical. Once the provider exam at FMC is completed, the physical is fully complete.There will NOT be an evening session at FMC.

Step 2: Complete MHSA Sports Physical Form
Use the QR code or the link provided to fill out the MHSA Physical form.
- Fill out the MHSA Sports Physical Form BEFORE arriving at Public Health.
- Forms will be pre-printed and ready for students upon arrival, but must already be completed.

Step 3: Complete Vaccination Form
Use the QR code or the link provided to fill out the Vaccination Consent form.
- Contact Public Health ahead of time to determine if your child needs any vaccinations.
- If your child is under 18, a consent form must be completed and electronically signed before they can receive ANY vaccines.
- This form must be completed BEFORE your appointment.
